1. Fill in the form on a laptop/ computer, please be as descriptive of the fault as possible.

2. Please attach an Adxba fault label to the equipment with the following information:
i. Airport (location)
ii. Job Number
iii. Unit Number and serial number
iv. Fault information
v. Who reported the fault
3. Package up the equipment and write the Job number on the Box
4. Send an email to returns@adxba.co.uk and attach the previously filled in form, in the heading of the email please also write in the job number.
5. Complete the online spreadsheet with the information you have

Inform ops on your end of shift report whether the equipment has gone back to the office with a Northern engineer or if postage needs to be arranged
